Well, you have covered about all that which might be the culprit. If you got a cheap DIC it could be bad out of the box. My guess is that you have a electrical issue and a ground short perhaps.

You may only have to tighten it and regrease it. Follow the shaft down below the dash till you see the connection. Separate the 2 ends, grease it and re connect it and tightened it down.
